Memorandum of Agreement (MOA)
between
OnStrike Library Solutions
and
Occupational Safety and Health Center Philippines
This historic MEMORANDUM OF AGREEMENT between the OnStrike Library Solutions, located
at 5 Capistrano St., Hagonoy, Taguig, represented by Mr. Jerwyn G. Fernandez, and the
Occupational Safety and Health Center Philippines, located at North Ave. cor. Agham Road,
Diliman Quezon City, represented by ____________________, recognizes the importance of
establishing a professional relationship that fosters mutual understanding, shared responsibilities
and a commitment to working together for the improvement of student learning.
I.
INTENT
THIS AGREEMENT is hereby made and entered into by OnStrike Library Solutions,
hereinafter referred to as the provider, and the Occupational Safety and Health Center
Philippines, hereinafter referred to as the client, who will collaborate to establish a Koha
based Integrated Library System for the latters library.
II.
SCOPE OF AGREEMENT
THIS AGREEMENT is intended to establish a Koha based library system for the clients
library which includes phone and e-mail support from the provider for a period of 1 year.
III.
DATE AND TERM
THIS AGREEMENT will become effective upon signing by all parties or in case of a
government project, the same will be effective on the date the provider receives the
purchase order of the said project. The Agreement shall be reviewed annually by the parties
to determine its continuation and/or need for modification.
IV.
PILC MEMBERSHIP
All OnStrike Library Solution clients will automatically be a member of PILC (Philippine
Integrated Library Consortium) and shall participate in resource sharing among its
members, which includes but not limited to OPAC sharing and the like.
V.
PARTIES MUTUAL RESPONSIBILITIES
Work cooperatively to ensure appropriate, efficient communication in support of the
objectives of this agreement.
Ensure consistent attendance by representatives at all meetings and functions related to
accomplishing the objectives of this agreement.
VI.
Regularly share information about the projects progress.

OFFSITE TESTING
UNLIMITED EMAIL AND PHONE SUPPORT FOR 1 YEAR**
TOTAL OF 46 DAYS
**Inclusive of regular system upgrade.
**1 year support and warranty starts the day after the training.
VII.
CLIENTS RESPONSIBILITIES
Appoint appropriate representatives of client to assist the provider in the entire agreement
process.
Facilitate participation by sharing resource/data that could help enhance the providers
service.
Contribute and share in the development of new Koha ILS, vis--vis not tampering with the
default set-up made by the provider that will cause error to the system.
Data back-up for redundancy in case of fortuitous event.
Assist in promoting Koha awareness in the community.
Provide a sever for the hosting of Koha ILS, with the following minimum system
requirements:

- 2GB RAM, or more if your server also runs a graphical desktop
- 500GB Storage
- 500GB Backup
- Linux Operating System
- Internet/Network connections
- Ports 22, 443, 8080, 10000 should be opened in your firewall for TCP connections
to allow OPAC and staff client connections
Payment of Php250,000.00 to the provider in a staggered basis not later than the date
provided for below (please refer to Section 6 for the phases):
