The undersigned, Atty. Oliver T. Misador would like to request for the CCTV
Footage captured in your system in San Antonio Valley, Paranaque Branch. So that
you may know, on 22nd of May 2018, at around 7:00 pm to 10:00 pm. The CCTV
footage obtained by the said camera/s are an important pieces of evidence which
may help us establish certain facts in relation to our fact-finding endeavor
involving our client, Janina Gallardo.

Previously, the parents of our client had already inquired and coordinated with
the management of said branch and fortunately, said video footages are already
viewed and available for retrieval. Given this premise, the undersigned would like
to formally request that the management provide us a copy of said footage for our
personal consumption and viewing.

Rest assured, the information gathered will not in any way prejudice or provide
any negative effects on Puregold Price Club, Inc., or any of its management or
employees but solely for the above-stated reason. Your cooperation will prove to
be indispensable and a favorable approval of the request is highly anticipated.
