emnly affirm and state on oath as follows.
1. I am the petitioner here in and as such I am well acquainted with the facts of the
case.
2. I respectfully submitted that, i am a member and resident of _______________
3. It is respectfully submitted that, when the Respondents failed to provide requisite
information to me, i filed W.P.No.____________ of 2020 and the Hon’ble High Court of
_____________, vide orders dated __________ in IA No. 1 of 2020 was pleased to direct
the Respondents more particularly ______and ________ Respondents to provide infor-
mation as requested by me.
4. It is respectfully submitted that, despite being parties to the writ proceedings and
having complete knowledge of the said order, the respondents have been conspicuous of
their in action for not acting upon the said order of the Hon’ble High Court. That despite
the subsistence of the Court orders, the respondents are unnecessarily precipitating the mat-
ter by unilaterally acting in contravention of the orders of the Hon’ble High Court in
I.A.No.______ of 2020 in W.P No.________ of 2020. That despite repeated requests from
me; no action has been forthcoming from the respondents.

5. It is respectfully submitted that, the respondents action in not complying with orders
dated __________ passed by this Hon’ble High Court amounts to wilful disobedience of
the order of the Hon’ble High Court passed in I.A.No.______ of 2020 in W.P No.________
of 2020. The narration of events would go to prove that the respondents are wilfully and
deliberately flouting the orders of this Hon`ble Court and they are liable for punishment
under the provisions of Contempt of Courts Act, 1971.
It is therefore prayed that the Hon`ble High Court may be pleased punish the re-
spondents with stringent punishment under Sec.10 to 12 of the Contempt of Courts Act,
1971 for not implementing the orders dated ___________ passed by the Hon`ble Court in
I.A.No.______ of 2020 in W.P No.________ of 2020 and to pass such other order or orders
as Hon`ble Court may deems fit and proper in the interest of Justice.
