1st: Paste link:

2nd: Click file and click MAKE A COPY:


3RD: You can rename it to anything you want. And press OK. It will generate new files in your
google drive folder and automatically open the saved file.

4th: the Import button is in Column L


5th: press the import button: it will open up some authorization message. Click the continue in the
authorization message:

6th: a popup will appear and click your email address.


7th: it will give you a warning. Click advance.

8th: after clicking advance additional message will appear: click “Go to sheetToForm (unsafe)”
9th: your will be redirected to: and click allow:

You have authorized are authorized to use sheetToForm script. Congrats.
