1. Request conduct Polygraph Examination on the following person who


were suspected responsible in stealing incident that transpired inside the premises of San
Yorkos Industries Philippines, Inc., Brgy Bacolong, Victoria, Tarlac on or about 6:35
p.m. of June 30, 2005 and on or about 5:45 p.m. of July 18, 2005, to wit:

a) Rizal Magno Y Mariano, 36 years old, married, SG of RubyGem


Agency and a resident of San Jacinto, Victoria, Tarlac;
b) Jonathan Gonzalo Y Fabros, 29 years old, married, factory worker
and a resident of San Nicolas, Victoria, Tarlac; and
c) Marlito Jose Y Javier, 36 years old, married, factory worker and a
resident of Maluid, Victoria, Tarlac.

2. Further request furnish this Office result of your examination for our
investigative reference.
