a.2. Basis for Prioritization
NURSING DIAGNOSES JUSTIFICATION
1. DECREASED CARDIAC A part of Maslow’s hierarchy of needs is airway, breathing,
OUTPUT and circulation (ABC), which are physiological elements
that are needed for the body to survive and help determine
one’s level of health. Decreased cardiac output is the most
priority and needs immediate attention while other diagnosis
can be delayed after this problem will be resolved.
2. ALTERED RENAL When following our ABC in nursing, Altered renal
PERFUSION related to perfusion is the next priority after decreased cardiac output.
glomerular malfunction This Altered renal perfusion is considered in the circulation;
also, it is 2nd priority in the MAAUAR, the impairment of
the urinary elimination. The patient should submit herself
immediately to hemodialysis in order to correct alteration in
renal perfusion.
3. IMBALANCED NUTRITION: This will be considered the next priority after altered renal
LESS THAN BODY perfusion is addressed because correction of the nutrition
REQUIREMENTS can be delayed but fluid volume excess together with
sodium retention and hypertension can be fatal if not
intervene immediately. According to Maslow, Nutrition is
part of the physiologic needs that should be met to survive.
4. RISK FOR IMPAIRED SKIN This is identified as the fourth priority nursing diagnosis
INTEGRITY because this is possible to become an actual problem due to
frequent itchy skin. If not prevented early and not managed
properly, it will lead to more serious problems.
5. RISK FOR ACTIVITY This is identified as the last priority nursing diagnosis
INTOLERANCE because this only occurs if interventions were not done
properly or if there were complication during the course of
treatment. Correction of blood lab levels should be done in
order to prevent anemia causing activity intolerance.
