DEDICATION

We humbly dedicate this case study to the whole faculty of ISABELA STATE UNIVERSITY
COLLEGE OF NURSING for raising academic excellence in the school and our dearest instructor in
Community Health Nursing for the knowledge and learning we gain from the very beginning, who
motivates us and believes in our academic capability, and who willingly and patiently taught us
everything we need to know in order for us to improve and achieve a quality study.

To the researcher’s very supportive parents, who always motivates us in doing our activities
and give us never ending support physically, financially and emotionally. To all the people who
compassionately helped us in conducting this study, who have shared knowledge and spare some of
their times in order for us to make this study in to reality.

And lastly, we dedicate this simple work to our Almighty God, the provider of strength and
author of knowledge; we offer this success all to Him.
