1. Define the key concepts of community and public health nursing;
The goal of community health nursing is to promote, protect and preserve the
health of the public. Community health nursing involves these basic concepts:
Promote healthy lifestyle
Prevent disease and health problems
Provide direct care
Educate community about managing chronic conditions and making healthy
choices
Evaluate a community’s delivery of patient care and wellness projects
Institute health and wellness programs
Conduct research to improve healthcare
1. Public health/community health nursing is a synthesis of nursing theory and public
health science. The basis for public health/community health nursing includes a
wide range of models and theories, such as: population health promotion and
primary health care (where the focus is on promoting and maintaining health),
illness and injury prevention, community participation, and community
development.

2. Enumerate the different levels of clientele in community and public health
nursing
The community is the patient in CHN, the family is the unit of care and there
are four levels of clientele: individual, family, population group (those who share
common characteristics, developmental stages and common exposure to health
problems – e.g. children, elderly), and the community.
3. Explain the various principles underpinning the practice of community health
nursing
The recognized need of individuals, families and communities provides the basis
for CHN practice. Its primary purpose is to further apply public health measures within
the framework of the total CHN effort. Knowledge and understanding of the objectives

and policies of the agency facilities goal achievement. The mission statement commits
Community Health Nurses to positively actualize their service to this end. CHN considers
the family as the unit of service. Its level of functioning is influenced by the degree to
which it can deal with its own problems. Therefore, the family is an effective and
available channel for the most of the CHN efforts. Respect for the values, customs and
beliefs of the clients contribute to the effectiveness of care to the client. CHN services
must be available sustainable and affordable to all regardless of race, creed, color or
socio-economic status.
CHN integrated health education and counseling as vital parts of functions. These
encourage and support community efforts in the discussion of issues to improve the
people’s health. Collaborative work relationships with the co-workers and members of
the health team facilities accomplishments of goals. Each member is helped to see how
his/her work benefits the whole enterprise. Periodic and continuing evaluation provides
the means for assessing the degree to which CHN goals and objectives are being
attained. Clients are involved in the appraisal of their health program through
consultations, observations and accurate recording. Continuing staff education program
quality services to client and are essential to upgrade and maintain sound nursing
practices in their setting. Professional interest and needs of Community Health Nurses
are considered in planning staff development programs of the agency.
Utilization of indigenous and existing community resources maximizing the success
of the efforts of the Community Health Nurses. The use of local available ailments.
Linkages with existing community resources, both public and private, increase the
awareness of what care they need what are entitled. Active participation of the
individual, family and community in planning and making decisions for their health care
needs, determine, to a large extent, the success of the CHN programs. Organized
community groups are encouraged to participate in the activities that will meet
community needs and interests.
Supervision of nursing services by qualified by CHN personnel provides guidance and
direction to the work to be done. Potentials of employees for effective and efficient
work are developed. Accurate recording and reporting serve as the basis for evaluation

of the progress of planned programs and activities and as a guide for the future
actions. Maintenance of accurate records is a vital responsibility of community as these
are utilized in studies and researches and as legal documents.
4. Identify the myriad roles and functions of the nurse in community and public
health nursing.
Healthcare Provider – Caring is the essence of nursing and has been widely
accepted in all settings. The use of nursing process applied in the natural
environment of the client pertains to this role.
Health Educator - Instructing patients how to take their prescribed medicine or
sharing information about the benefits of healthy lifestyle illustrates this role.
Program Implementer - Nurses working under local government units deliver
healthcare to the grassroots. This also requires that programs initiated by the national
government thru the Philippine Department of Health (DOH) are executed at all levels. -
this includes immunization which offers protection against infectious disease. - nurse
works with the rural health midwife in conducting routine vaccination in communities to
achieve herd immunity and contribute to the over-goal of reducing child mortality
Community Organizer - Working with the people in communities and
providing them ownership of their healthcare needs and to act collectively on their
issues is best captured by this role of the nurse in public health.
Manager/Leader - Community and public health work deals with competing
claims specially on utilization of limited resources.
Researcher/Epidemiologist - The nurse is regarded as the health monitor of
the community. Together with the rural health midwife, they keep track of illness
encountered in their areas of jurisdiction and submits reports to health authorities as
required by law.
Client Advocate - The CPHN takes a proactive stance in ensuring that the right
to health of the population he/she works with is realized and protected. - the nurse in
this field initiates organizing activities to form coalitions and linkages to foster
awareness of people to various health-related issues.

5. Discuss the various public health interventions applied in practice.
Public health nurses focus on the care of individuals, groups, aggregates, and
populations in many settings, including homes, clinics, worksites, and schools. Public
Health nurses must also work with community to identify and implement programs that
meet health needs and to evaluate the effectiveness of programs after
implementations. Interventions are actioning that public health nurses take on behalf of
individuals/families, communities, and systems, to improve or protect health status.
This framework, known as the Intervention Wheel, defines the scope of public
health nursing practice by type of intervention and level of practice (individual/family,
community, or systems), rather than by the site of service such as home, school,
occupational health, clinic, and others.
