i. TITLE: 15 – DAY DISTRICT- BASED DEVELOPMENT AND QUALITY ASSURANCE
EVALUATION OF LEARNING ACTIVITY SHEETS (LAS) IN SECONDARY AND
ELEMENTARY SCHOOLS AT DONSOL WEST 2 DISTRICT FOR SY 2021- 2022

1- RATIONALE:
The Department of Education has taken a big step through DepEd Order No. 12 s.
2020 entitled Adoption of the Basic Education Learning Continuity Plan SY 2020- 2021 to
change the delivery of the traditional face to face instruction to alternative forms to ensure the
continuity of education amid pandemic.
Now, on its second year of the LCP, accelerated changes in the ways of delivering
quality education are given emphasis to update the new teaching methods delivery and
modality. The use of Learning Activity Sheets is perceived to be a child- friendly resource in
home- study sessions. Thus, this district is supportive of the SDO’s initiative to prepare
educational materials that are MELC- Based, in ways facilitative to the needs of the learners.
Objectively, the activity will;
Discuss the formulation of LAS and identify its importance.
Develop a contextualized and localized LAS following necessary processes and
procedures.
Provide learning materials before the opening of the classes for SY 2021- 2022.
II- PROCEDURE/ METHODOLOGY
Phase 1:
Secure approval of the AR/ATC and Proposal
Conduct meeting with the SHs and key teachers (writers, subject coordinators,
QA Team)
Prepare the deliverables (venue, materials, personnel, etc)
Phase 2:
Conduct the 10- Day LAS Development in Elementary and Secondary Schools
Schedules of work accomplishments (write shop and QA processes)
Revisions after Final QA
Phase 3:
Reproduction of LASs
Awarding of Certificate of Recognition (writers and QA Team)

Materials (bond papers, ink, laptop, MELC, other sources)
Financial (Meal, transportation and materials used)
Human (reimbursements)
All expenses incurred in the conduct of this activity is subject to the usual accounting
and auditing rules and regulations.
