Dear Respondents,

We are students of Lyceum of Subic Bay under the Science, Technology, Engineering,
and Mathematics (STEM) strand. Our research group will be conducting a study entitled, “The
Teaching of Sex Education Among Junior High School Students at Lyceum of Subic Bay as
Perceived by Selected Senior High School Students, A.Y. 2020-2021” In partial fulfillment of
our subject, Research in Daily Life 2. If you consent to participate, we will provide you with
the study’s survey questionnaire and we guarantee that all data gained from you will be handled
with strict confidentiality following research ethics. We appreciate your consideration and

of sex education.

early sexual debut.

D. Sexually Transmitted Diseases or STDs

the common symptoms of STDs.

to learn about how risky STDs are.
