sections: R, I, A, S, E, C.

• For each question, you are asked to respond with a “yes” or “no” answer. Add up all your “yes” answers at the
end of each section.
• Transfer the six totals from each of the three sections to the three rows on the scoring grid. Add up the three
numbers for each of the six columns and insert the six totals. Enter the letters that correspond with the first,
