DEF: Your Honor, the testimony of the witness is being presented for the purposes stated in the Judicial Affidavit. We will adopt the
same.
DEF: Ms. Witness, do you recall having executed a Judicial Affidavit in relation to the present case?
DEF: I am now showing to you this Judicial Affidavit consisting of 6 pages. Is this the same Judicial Affidavit that you executed?
W: Yes, ma’am.
DEF: Now, on page 5 of the Judicial Affidavit, there appears a signature above the printed name Corazon Balbag Evangelista, whose
signature is that?
W: Mine, ma’am.
DEF: Your Honor, we will be presenting additional documentary evidence - the marriage certificate of the witness and the petitioner’s
brother. This was not previously marked since the present witness was not originally going to testify.
DEF: Now, Ms. witness, in your Judicial Affidavit, you made mention of you and your husband’s marriage certificate. I am showing to
you the original copy of the said marriage certificate, is that the same document that you identified in your Judicial Affidavit?
W: Yes, ma’am.
DEF: In this marriage certificate, under the column “husband” there appears a name “___________”, is this the husband, who is the
brother of the petitioner, that you were referring to in your JA?
W: Yes, ma’am.
DEF: In this marriage certificate, there appears a name “___________”, is this you Ms. Witness?
DEF: Your Honor, we will be submitting the photocopy of the said marriage certificate. But I have with me the original. May I move that
the document be compared with the original as faithful reproduction?
DEF: May we request that the photocopy of the marriage certificate be marked as Exhibit “L”. And that the name of witness be bracketed
and marked as Exhibit “L-1” and the name of the witness’ husband’s name be marked Exhibit “L-2”.
DEF: Mr. Witness, you also made mention in your Judicial Affidavit a Deed of Donation. I am confronting you with this document
marked “Exhibit G”, is this the same Deed of Donation that you mentioned in your Judicial Affidavit?
W: Yes, ma’am.
DEF: Ms. Witness in your Judicial Affidavit you also mentioned of a Certificate Authorizing Registration. I am showing to you a
document of what appears to be issued by the Bureau Internal Revenue, marked as “Exhibit H”. Is this the same Certificate Authorizing
Registration that you mentioned in your Judicial Affidavit?
W: Yes, ma’am.
Now, Mr. Witness, have you read this Judicial Affidavit of yours?
W: Yes, ma’am.
DEF: Do you confirm and affirm that the contents hereof are true and correct and based on your personal knowledge or authentic
records?
W: Yes, ma’am.
DEF: Your Honor, with the identification of the Judicial Affidavit by the witness, we move that the same be considered as her direct
testimony.
