RELEASE, WAIVER, AND QUITCLAIM
KNOW ALL MEN BY THESE PRESENTS:
I, Juan del la Cruz, of legal age, single, and a resident of # 123 Main St.,
Malate, Manila, after having duly sworn to in accordance with law hereby
depose and state:
1. I hereby acknowledge receipt of __________(Php_______) (the
“Consideration”) from __________________;
2. I acknowledge the sufficiency of the Consideration as sole
consideration for Release, Waiver, and Quitclaim;
3. I voluntarily accept the Consideration for the purpose of creating a full
and final settlement of any and all claims for any losses, damages,
injuries, or otherwise that may arise from the following transaction (the
“Transaction”)________________;
4. In view of the receipt of the Consideration, I hereby quitclaim, release,
waive, and discharge, forever and unconditionally,
____________and/or their agents, representatives, and successors-in-
interest from any and all causes of actions, sums of money, claims,
damages, demands, and any further liability whatsoever in law,
contract, and equity, past, present or contingent, which I or my
successor-in-interest, representatives, assigns had, have, or may have

successors-in-interest in connection to or arising from the Transaction;
5. I shall not file nor cause to be filed any legal action or complaint in
connection with or arising from the Transaction in any office, board,
bureau, court, tribunal, and agency against________and/or their
agents, representatives, and successor-in-interest, representatives, and
assigns will no longer participate, testify, prosecute, or provide any
evidence in said action;
6. I shall likewise not participate, testify, provide any evidence, or in any
way assist in any and in all actions or complaints in connection with or
arising from the Transaction in any office, board, bureau, court,
tribunal, and agency against _______________and/or their agents,
representatives, and successors-in-interest;
7. I acknowledge that this Release, Waiver, and Quitclaim shall not be
construed as an admission of any wrongdoing by ______________in
relation to me or any other person or entity;
8. I likewise acknowledge that this Release, Waiver, and Quitclaim shall
not be construed as an admission of liability by ___________nor does
it indicate that I have any actionable right against ____________;
9. I hereby state that I have read this document and have fully understood
its contents. I further declare that I voluntarily and willingly executed
this Release, Waiver, and Quitclaim with full knowledge of my rights
