Questionnaire
As part of your consultation with the client, you will want to ask a series of
questions to give you a better idea of how many personal training sessions
they will need as part of their as part of their onboarding phase.

These are not the only questions you’d ask the client, but these are the
questions that will give you a quantitative answer on the start to their journey.

1. What best describes your experience with fitness and training based on
the following:

I’ve never done weight training or functional + 9 sessions

*For ease of use, when working with 10 or more sessions, subtract 4 PT
sessions from the total number needed to determine when the client moves
into group sessions. The 4 sessions you subtracted are their 2 ghost sessions +
2 future individual sessions. The intention of this questionnaire is for the
person who is a fit for group class. If they are not a fit and purely need PT for
an extended period of time, you will not use this formula, as they will need
more sessions than it will tell you.

Notes:
• Answers to question number 2 could be described to you or found on
the movement screen. Use these sessions to test the capacity of the
impairment to handle intensity and recover.
• The value of adding sessions based on question number three is
another opportunity to groove RPE increases.
• Each added session from question number five serves the same utility
as sessions added from question 3

Now, tally up the amount of sessions they need to get started. As we said
already, most people will need at least 12 sessions, and typically more.

As an incentive to sign-up in the consult, you can offer these sessions at the
24 pack rate instead of at the single session or 12 pack rates.

Here is an editable template with this questionnaire that you can use in your
consultations.
