Further to the interview held with you and our subsequent discussions, mass medical mission (mmm) is delighted
to offer you employment in Health Plaza Abuja.

Health Plaza Abuja is a multipurpose nonprofit initiative aimed at supporting the free health missions of mass
medical mission, towards improving the health indices of the citizens.

CONTRACT OF EMPLOYMENT
1. POSITION: You have been appointed as GENERAL ASSISTANT of Health Plaza Abuja.

2. DURATION: Your employment shall commence on Monday 25 September 2023 and shall continue from that
date, unless terminated in the manner contained in this employment contract.

3. PROBATION: You are required to serve a probationary period of six months, after which your service will be
confirmed in writing, subject to satisfactory performance. Therefore, the probationary period may be extended or
shortened based on your performance.
4. PLACE OF EMPLOYMENT: You will be located at the Health Plaza Abuja.
5. DUTIES: Your specific duties shall be discussed upon your assumption of duty. These include but are not limited
to: housekeeping, laundry, driving, sending daily reports, and generally assisting to ensure the smooth running of
the Apartment. Your duties and benefits are subject to periodic review and can be changed with or without notice,
based on your performance.
6. HOURS OF EMPLOYMENT: Your duty schedule and hours of work shall be as prescribed from time to time
by the Organization. Notwithstanding the hours of employment communicated to you, you may be required to
work beyond these hours during weekdays, weekends and public holidays as may be necessary. Therefore,
overtime allowance is included in your consolidated remuneration.

7. REMUNERATION AND PERQUISITES: Your monthly remuneration and perquisites shall be a consolidated
amount of SEVENTY THOUSAND NAIRA ONLY (N 70,000.00), out of which fifty percent is fixed and fifty
percent is performance-based. Your performance goals will be determined from time to time by the management.
The breakdown of the fixed is as follows:

iii. Hazard/Inconvenience allowance 10%
iv. Housing allowance 15%
v. Leave allowance 10%
vi. Medical allowance 10%
vii Out-of-station allowance 10%
viii. Overtime 15%

8. INCOME TAX & PENSION: After confirmation, you will be liable to pay personal income tax and pension.

9. ANNUAL VACATION: You shall be entitled to fifteen (15) calendar days’ annual vacation with pay. Your first
vacation will become due after you have completed thirteen (13) months of employment. Your leave shall be
taken at a time that is least disruptive to the operation of the Organization.

10. DISCIPLINE: Lateness, absence without permission and any other forms of misconduct are not acceptable under
any guise and shall attract monetary penalty.

11. LIMITATION: You shall not utilize the Apartment as residence for ANYONE whatsoever under any
circumstances, including yourself, co-workers or agents. For the avoidance of doubt, neither you nor anyone else is
permitted to pass the night in the Apartment (except if on night duty), without the Owner’s written permission.

12. TERMINATION OF EMPLOYMENT: To avoid undue disruptions of its mission, the Organization requires
passionate, dedicated and stable workers. Therefore, you are required to give one months’ notice of termination of
employment in writing or one months’ salary in lieu of notice. For the avoidance of doubt, if you exit the
Organization (under any circumstances) without the statutory notice, you shall forfeit any outstanding salary.
13. SUMMARY TERMINATION OF EMPLOYMENT: The Organization may terminate your employment without
notice, indemnities and compensation if:
i. you are, in the opinion of the Organization, guilty of gross negligence, dishonesty, or misconduct;
ii. it is discovered at any time after your employment that any piece of information you supplied during or after the
process of the job interview is false or that you deliberately withheld vital information;
iii. you are found to have made illegal monetary profit or received any gratuities or other rewards, in cash or in
kind, out of any of the Organization’s affairs or any of its subsidiaries or affiliates;
iv. you are guilty of serious or repeated non-observance by you of the Rules and Regulations of the Organization;
v. you are convicted of a criminal offence other than a Road Traffic offence;
vi. you are unable/incapable of performing your duties for an aggregate period exceeding six (6) weeks in one year.

14. LAST SALARY: When you leave the Organization for any reason whatsoever, your last salary (where applicable)
will be paid not earlier than the last day of the month that follows your last month at work. Where applicable, you
may be required to fill an exit clearance form and undergo an exit interview before your last salary is paid.

Following the termination of your employment, any property of the Organization in your possession) shall be
surrendered on or before your last day at work. Failing this, your last salary and other entitlements could be used to
make good any losses or inconveniences thereby incurred, as determined by the Organization.

The above terms relating to the payment of the last salary and handover of the organizational property also apply if
you are placed on suspension. For the avoidance of doubt, if for any reason, you are placed on suspension, you shall
not be entitled to salary during the period of suspension.

15. OTHER TERMS & CONDITIONS

i/ This foregoing offer of employment is contingent upon satisfactory verification of any information furnished under
your personal data; and good recommendation from your references. The Organization reserves the right to conduct
background checks to verify any of your claims, including contacting your prior educational institutions and employers.

iii/ Your first salary will be due only after you complete the employee data form and submit your credentials as well as
your duly signed and notarized guarantors’ forms, which will be sent to you after you accept this offer of employment.

iv/ You acknowledge that this contract of employment, represents the entire agreement between you and mass medical
mission and that no verbal or written agreements, promises or representations that are not specifically and explicitly
stated in this employment offer letter, are or will be binding upon mass medical mission.

v/ It must be noted that, without prejudice to the generality of the foregoing terms and conditions, the Organization
reserves the right to add to, vary or abrogate any of the provisions contained herein with or without notice if, in the
opinion of the Organization, it is necessary or desirable to do so.

16. ACCEPTANCE: Please study the contents of this Contract of Employment carefully and if you are
satisfied therewith, sign below and return a scanned copy of this page by email to email@healthplaza.org within
5 days, failing which this offer of employment will lapse. A photocopy of this contract should be submitted to
mmmHouse upon your assumption.
