 DATA PRIVACY POLICY AND BACKGROUND CHECK CONSENT FOR JOB APPLICANTS

In the course of determining whether to hire you for employment purposes, DITO Telecommunity Corporation will collect and process the Personal
Data you provided above, which includes information for identification and other personal circumstances, contact information, employment
preferences, educational and employment background, personality attributes, skills, licenses, certifications, and other credentials, organizations and
affiliations, and government-issued information and/or certifications or clearances. In addition to the Personal Data you provided in this application
form, DITO Telecommunity Corporation will collect Personal Data thru various channels such as third party background checks, interviews, and
online tests, as well as assessments you may have personally completed.

The collection of your Personal Data and disclosure to third parties (where required or necessary) will be for the following purposes:

• Assessing and evaluating your suitability for employment in any current or prospective position within DITO Telecommunity Corporation;
and
• Verifying your identity and the accuracy of your personal details and other information provided.

The Personal Data you provided will be retained in DITO Telecommunity Corporation’s records as part of its potential talent pool for a period of five
(5) years, (“Personal Data Retention Period”). Unless retention for a longer period is required for reasonable cause, the disposition of Personal Data
shall, upon the lapse of the Personal Data Retention Period, be effected in accordance with applicable laws and regulations.

You are entitled to certain rights in relation to the Personal Data that may be collected from you, including the right to access, correction, and to
object to the processing, as well as the right to lodge a complaint before the National Privacy Commission in case of violation of your rights as a data
subject.

(a) You acknowledge that you have read, understood and agree with the above Policy and Consent Form, and consent to the collection,
processing, use and/or disclosure of your Personal Data by us for the purposes set out in the Policy and Consent Form;

(d) You authorize DITO Telecommunity Corporation and its affiliate/s to collect, store, and process your Personal Data in connection with existing

(e) You agree to update your personal information to keep your Personal Data current, complete, and accurate and when so required by DITO
Telecommunity Corporation or its affiliate/s;
(f) You authorize DITO Telecommunity Corporation, its affiliates, and any authorized third-party background check provider to conduct a
background check (which includes, but is not limited to: education, employment, credit, neighborhood or criminal checks) and/or reference
checks. For this purpose, you further authorize DITO Telecommunity Corporation and its affiliate/s to share any personal information with
such third-party when necessary in conducting the background check.
(g) You acknowledge that you shall render yourself liable for any false information that you may have provided and that DITO Telecommunity
Corporation has the right to withdraw or cancel any form of job offer or contract of employment should there be any discrepancy found with
