Instructions:​
Read each scenario and assign the most appropriate CPT code. Use your CPT manual or a
trusted coding resource.

Evaluation & Management (E/M) Services

1.​ Established patient visit for diabetes follow-up with moderate medical decision-making.​

2.​ New patient physical exam, age 35, no complaints.​

3.​ Emergency department visit for chest pain with high-complexity decision-making.​

4.​ Established patient visit with a problem-focused history and exam for a sore throat.​

5.​ Telemedicine visit for medication management, moderate complexity.​

Surgery – Integumentary System

6.​ Simple repair of a 4.0 cm laceration on the upper arm.​

7.​ Excision of a benign skin lesion on the back, 1.2 cm.​

8.​ Removal of impacted cerumen, both ears, using irrigation and instrumentation.​

9.​ Cryotherapy of two common warts on the hand.​

10.​Incision and drainage of a small abscess on the thigh.​

Radiology

11.​Chest x-ray, two views, frontal and lateral.​

12.​CT scan of the abdomen and pelvis, with contrast.​


13.​MRI of the brain, without contrast.​

14.​Screening mammogram, bilateral.​

15.​X-ray of the right foot, 3 views.​

Pathology & Laboratory

16.​Complete metabolic panel (CMP), routine.​

17.​Rapid strep test in the office.​

18.​Urinalysis, non-automated, without microscopy.​

19.​Hemoglobin A1c test.​

20.​Pap smear with HPV testing, routine screening.​

Medicine

21.​Influenza vaccine administration, intramuscular.​

22.​ECG with 12 leads, including interpretation and report.​

23.​Spirometry, pre- and post-bronchodilator.​

24.​Physical therapy evaluation, moderate complexity.​

25.​Therapeutic injection (e.g., corticosteroid) into the right shoulder joint.​
