I. Executive Summary (Description of why the school is deserving of the


award in not more than 150 words which includes TEA Governance
implementation, financial management, DepEd programs and Projects
Implementation.)
Patabog Elementary School has demonstrated unwavering commitment to TEA
(transparency, ethical, accountability) Governance in its school management, consistently
guaranteeing that every resource is allocated responsibly, used effectively and liquidated
honestly benefiting its personnel and students towards improved performance, hence, a
deserving candidate for this prestigious award.
Its dedication to implementing its PPAs aligned with SDO Quezon’s banner programs
and that of DepEd MATATAG is noteworthy as shown in the decrease of nonreaders in
English from 42% to 4% or 191 to 20; Filipino from 17% to 4% or 78 to 17; non-numerates
from 48% to 11% or 131 to 48 from Grades 1-6; and an increase of MPS from 55.24% to
70.27%.
Its initiatives on continuous professional development for teachers led to the
improved teacher performance with Outstanding Rating from 42% or 8 to 78% or 14 out
of 18 teachers geared towards enhanced teaching practices and learner performance.
II. Significant Accomplishment/s within the last two years (SY 2021-2023)

Over the past two years (SY 2021-2023), Patabog Elementary School has
achieved small yet remarkable milestones through the implementation of innovative projects,
programs, activities, and strategies under its Learning Continuity Plan (LCP) 2021-2022
and Hazzard-free Learning Environment, Enhancement of Literacy and Numeracy,
Advancement of Teachers/ Competence, and Linkages of Stakeholders (HEAL) -
Learning Recovery Continuity Plan (LRCP) 2022-2023 anchored on the four pillars of the
Basic Education Development Plan (BEDP) on Access, Equity, Quality, and Resilience and
Well-being. All the efforts exerted resulted in the improvement of our learners and school
performance, to wit:

school. For SY 2022-2023 however, Penulat ni Juan is used to give


rewards to learners who have perfect attendance every quarter. The
goal is to keep learners from frequent absenteeism.

Description An innovation for inclusivity where identified disadvantaged learners are
accommodated and given appropriate interventions based on their
needs.
As surfaced during the staff meeting, teachers observed ten learners
with manifestations of learning disability despite their efforts to provide
intervention for them. This prompted the proponent to initiate the
handling of the special program for these learners. They were
mainstreamed to their appropriate level and given further intervention in
the afternoon through pull out session with the reading teacher.

Recovery Plan 2: Enhancement of Lieracy and Numeracy Skills,
aligned and modified from Mulanay STARR of the district to combat the
problem of emerging number of non-readers and non-numerates. It

least 50% at the end of the school year; use prototype lesson plan in
teaching reading using Marungko approach; use explicit teaching in
developing lessons in reading and numeracy; and design rubrics per
