INSTRUCTIONS TO THE STUDENTS

1. Official Transcripts is a consolidated statement of marks of all examinations, which is issued by the
University in the prescribed format and is issued only in the name of Agency/Embassy/Educational
Institutions/Universities/Credential Evaluation Services where the student is seeking employment/higher
education/Immigration purpose, etc. No unofficial transcripts or transcripts without the address of will be
issued.
2. Official Transcripts will be issued in sealed envelopes only to the address mentioned in the application or
will be directly forwarded to the official email id of the addressee.
3. In the case of WES – World Education Services the soft copy of the Official transcripts will be directly
upload to the WES portal. Students shall mention the WES Reference Number correctly.
4. Duly filled Academic Records Request Form has to be forwarded to WES/IQAS with the Official
Transcript. Hence, the student shall submit the same along with application for official transcript after
filling the top portion of the form and put his/her signature at appropriate column. (if applicable)
5. Fee once paid is not refundable or adjustable under any circumstances. Students shall be vigilant
while making the payment. Don't pay any extra amount apart from the requisite fee.

7. Remittance of Fee
Fee can be remitted at University cash counter between 10:00 am to 03:00 pm or can be paid by way of
Demand Draft (DD) or by online transfer.
If payment is by way of Demand draft;
the DD should be drawn in favour of: The Registrar, CUSAT
payable at: (a)If drawn on SBI – Cochin University Campus branch (Code: 70235)
(b) If drawn on any other bank – Ernakulam.
the DD should be enclosed along with the application form

8. Submission of Applications
Completed applications shall be directly submitted to the Office of the Controller of Examinations or sent
to the address - Controller of Examinations, Administrative Office, Cochin University P.O, CUSAT,
Kochi – 682 022, Kerala, India or by email to controlexcusat@gmail.com (Subject line as: Application
for Official Transcripts in r/o <Name of Student>, <College Name>) with the following enclosures
(a) Proof for payment of fees
(b) Copies of all Marklists
