PETITION
COMES NOW, the herein petitioner through counsel and unto the
Honorable Court, most respectfully states:
1. Petitioner is of legal age, Filipino, married, and a resident of
Dumanlas, Buhangin, Davao City, where he maybe serve with summons and
other processes of the Honorable Court.
2. Named nominal party defendant is the Office of the Local Civil
Registar, City of Tagum, with address at Tagum City Hall, Tagum City, in
charge of registration of birth, where it may be serve with summons and
other processes.

3. The Certificate of Live Birth of petitioner contained erroneous data,


hereunder indicated, compared with the correct one:

B] Date of Birth: Feb. 28, 1992 Feb. 29, 1992

4. Petitioner needs to correct the erroneous entries, to conform with the


data he used in all records of his dealings with the government.

5. Attached are the following documents to support the petition:

Annex “A” - NSO Copy of Certificate of Live Birth of Socrates


Cruz y Quambot; and the Receipt.

Annex “B” & “B-1” - Marriage Contract of Petitioner indicating


Correct Entries and Receipt.

Annex “C”, D” & “E”- UMPC ID; Professional Driver’s License; and
TIN No. 123-456-789 of petitioner indicating
Correct name and date of birth.

PRAYER

Premises considered, it is most respectfully prayed of the Honorable


Court, that after trial:

1. An order be issued directing the Local Civil Registrar, Tagum City


and the Philippine Statistics Authority (PSA) to effect corrections on the
Certificate of Live Birth of Petitioner as follows:
