Key Root Cause Analysis is a method of problem solving used for

Introductory Present the slide on iceberg phenomena and ask volunteers to

Present slide presentation No. 4, 5 and 6 on the introduction of
the Root Cause Analysis

Group Activity

Instructions :
1.Revisit your SIP for School Year 2016-2019.

2. Write on a Manila paper the problem/problems that surfaced on


the implementation of your SIP and try to identify the causes.

3. Present your outputs to the group.


After the reporting, present slide No.9

The whole class discusses on how to use the different problem


analysis tools.

Note to the Facilitators: You may use sample Why Why ,Tree and
Fishbone Diagram in your discussion .(see attached appendices)

Let the participants answer again the following questions:


• What is Root Causes Analysis?
• What are the different methods or tools used in
identifying the root cause of a problem?
Application Let’s Walk the Talk
(2 hours )

1. Fill –up the 5 Why’s, Tree and Fish Bone Diagram template
provided using your own identified problem.

Note to the Speaker: Please prepare the different templates (it


should be enlarged) by placing them on a Manila Paper.

2. Use the following meta cards in posting the answers


SH - white
Teacher - yellow
Learner - Pink
PTA - green
Ed. Com. – blue

Note to the Speaker: Call each group to present their outputs and
guide the reporter during the processing of their answers.

After the presentation, tell the participants that after identifying the
root cause the next step is to present it to the SPT/PT
