The purpose of an academic

Below are the terms to which the Student must agree to participate in the internship for credit.
The Student must sign at the end of these terms.

TERMS AND CONDITIONS AGREED UPON BY THE STUDENT

I, the Student:
A. Will intern for the Internship Supervisor listed above for the noted semester to the best of my ability.
B. Will always conduct myself professionally including but not limited to: arriving to the internship on-time, completing minimum 75 hours at the internship, and
upholding any designated responsibilities, the internship site’s dress code and code of conduct.
C. Will work the agreed upon number of hours, noted above, unless I have given my Internship
Supervisor reasonable notice or there is an emergency.
D. Will meet all the requirements of the internship that I agree upon with my Internship Supervisor and my Career Advisor.
E. Will report worksite issues or problems to my Internship Supervisor and my Career
Advisor immediately.
F. Will immediately report to my Career Advisor if my internship ends, for any reason, prior to the scheduled end date.
G. Understand that I am financially responsible for all courses for which I register and that any changes to my registration may impact both my financial aid
eligibility and my balance due to the college university or institution I attend.
H. Understand that any other registration changes (add, drop, or withdraw) must follow the deadlines on the Office of the Registrar website.
I. Understand that if I am fired from my internship, it may negatively impact the overall grade of my internship course.
J. Understand that final grade for the Internship course is based on my communication with my Internship Supervisor and my Career Advisor, Faculty (if relevant),
the Internship Evaluation completed by the Internship Supervisor and the writing requirements set forth by my Career
Advisor and Internship course.
K. (For International Students ONLY): I understand I must complete the necessary paperwork with the Office of International Student & Scholar Services in order
to receive authorization for CPT
(internship). A new I-20 for CPT must be issued by proper authority or Designated School Official before I begin my internship, even if the internship is unpaid.

INTERNSHIP SUPERVISOR AGREEMENT TERMS

TERMS AND CONDITIONS AGREED UPON BY THE INTERNSHIP SUPERVISOR

I, the Internship Supervisor:


A. Have read and understand the policies and procedures set forth by U.S. Intern Guidelines as defined in the Employers Guide to Internships and Best Practices
and
Employer Policies.
B. Agree that the information designated above is accurate and I will provide the necessary number of hours needed and the educational content for the student
to complete his/her internship.
C. Commit to the creation of useful opportunities for the Intern to strengthen the workplace skill(s) and to offering the Student the minimum 75 hours needed to
complete his/her internship for credit.
D. Agree that this internship will provide the following learning objectives.
The Student will:
• Develop a practical understanding of the company’s industry.
• Become familiar with working with in a professional environment or with industry professionals.
• Recognize how to apply principles the Student has learned in their coursework or on their own to the workplace.
• Understand what a student learned in their internship and how those skills can be applied to their post-graduate career.
E. Inform the Career Advisor of any major changes to the internship. This can include but is not limited to: the student resigned from the internship, change in
Internship Supervisor, the student doesn’t show up for the internship for several days in a row, and the student is performing exceedingly poor.
F. Prepare the Student for any significant change that may happen during the internship and communicate this change to the Career Advisor immediately. This
can include but is not limited to: change in intern’s responsibilities during the internship or change in Internship Supervisor.
G. Comprehensively complete the Supervisor’s Evaluation of the Intern and return it to Career Advisor before the deadline at the end of the semester. This
evaluation will help determine the Student’s final academic grade in the internship course.
H. Understand that the Career Advisor has the right to request a site visit at a mutually determined time and date.
I. Recognize that the internship is tied to the Student’s formal education and that the internship must accommodate the Student’s course schedule and academic
obligations.
