ADDENDUM
TO CONTRACT OF LEASE
KNOW ALL MEN BY THESE PRESENTS:
This Addendum to the Contract of Lease, made and entered this ___ day of
November, 2018 at Batangas City, Philippines, by and between:
_____________, a corporation duly organized and existing under
Philippine laws, with business address at ______________________, of
legal age, Filipino and with residence address at _________________,
hereinafter referred to as the "LESSOR";
- and -
_________________, a sole proprietorship, with office address at
Blk 17 Lot 10 RL Carmel Height, Kayumanggi, Lipa City, Batangas, and
registered under the name of _____________________, of legal
age, Filipino and with residence address at
____________________,hereinafter referred to as "LESSEE";
WITNESSETH THAT:
WHEREAS, the above named parties executed a Contract of Lease on September 3,
2018, at Batangas City, which was entered as Doc. No. 342, Page No. 70, Book No. XIX,
Series of 2018, in the Notarial Register of Notary Public Atty. Dianne Ruth B. Aquino – De
Sagun;
WHEREAS, the parties hereby agree under Section 1 of the Contract of Lease to
wit:
“Section 1.SUMMARY OF BASIC LEASE PROVISIONS– It is expressly agreed that the floor
area of the leased premises shall be SEVEN (7) SQUARE METERS, and that the rental rate
shall be read as follows, to wit:
BASIC MONTHLY RENT Php18,000.00 plus VAT 4th month onwards
DISCOUNTED RENT Php13,500.00 plus VAT 1st to 3rd month of lease
WHEREAS, theabove named parties agree as they do agree to amend the
previously executed Contract of Lease to reflect the true and real intention of the parties;
NOW, THEREFORE, for and in consideration of the foregoing premises, the parties
hereto have agreed as they do hereby agree as follows:
1. The WHEREAS Clauses under Section 1 of the Contract of Lease should be amended as
follows:
“Section 1.SUMMARY OF BASIC LEASE PROVISIONS – It is expressly agreed that the floor
area of the leased premises shall be FOURTEEN (14) SQUARE METERS, and that the rental
rate shall be read as follows, to wit:
BASIC MONTHLY RENT Php36,000.00 plus VAT 7th month till 12th month
DISCOUNTED RENT Php27,000.00 plus VAT First six months of lease
2. The stipulations and agreements stated in the other provisions of the Contract shall

remain the same unless sooner amended by both parties.
IN WITNESS WHEREOF, the parties herein have hereunto set their hands at
Batangas City, this _____ day of November, 2018.
