PARAMETER NOTES EXAMPLE


ECG Strip Boxes
1 Large Box = 5 Small Boxes
1 Large Box = 0.20 sec
1 Small Box = 0.04 sec

Standard
Speed = 25 mm/sec
Amplitude = 10 mm/mV
Rate REGULAR RHYTHM (see Lead II) NV: 60 – 100 bpm
1. 5 Small Box Approximation
300 – 150 – 100 – 75 – 60 – 50
2. HR Formula
Heart Rate = 1500 / # of Small Boxes in R – R

IRREGULAR RHYTHM (see Lead II)


1. 6 Second Strip
Count # of R in 30 Big Boxes
30 Big Boxes * 0.20 sec = 6 Seconds
Heart Rate = # of R in 30 Big Boxes x 10

Rhythm Sinus Rhythm:


Downward in aVR
Upward in Lead II, III, aVF
Presence of P Wave
PR Interval # of Small Box from P to Q x 0.04 sec NV: 0.12 – 0.20 sec
3-5 Small Squares
SHORT: Accessory Pathway
PROLONGED: AV Block
QRS Complex NV: 0.08 – 0.10 sec
or < 12 sec

Axis Formula
Axis = (90 x avF) / ( |Lead I| + |Lead aVF| )
Finding aVF or I Value:
1. Identify QRS Isoelectric Point
2. Count Small Boxes of R going up
3. Count Small Boxes of R going down
4. aVF “R” Up + aVF “R” Down = Lead aVF Value

IF Lead I is NEGATIVE:
180 – Axis = Final and Corrected Axis Value
