PAYROLL DEDUCTION
Allowed under RSA 275:48 I

I, Jastine C Pamintuan herby authorize Channel Information


Technology Support and Leasing Services, Corp. (d.b.a BPO
Seats) to deduct from my wages for SSS Salary Loan the sum
of 40,000, beginning 01/21/2022 and ending 01/21/2024 until
the amount of 36,038.40 has been deducted.

I am authorizing this voluntary deduction as specified in RSA


275:48 I.

In the event my employment ends for any reason before the


final deduction, the entire balance may not be deducted from
my final wages.
