Individual Assignment (10%)

Requirements

As a management trainee in Ultimate Gym and Fitness Center, you have been asked to
develop a simple database to store information of the members. You need to design a
database which consists of a table, a form, three queries and three related reports.

The Task

The owner would need a customer list (containing all the necessary contact details and
other personal information such as member’s number, last name, first name, address,
contact number, gender, date of birth, blood type, types of training, name of instructor,
monthly member’s charges, and occupation). Assign one field to be a primary key for the
table. Enter at least 20 customer records (10 female customer and 10 male customer)
using the form that have been created specifically for this database.

Define the structure of the table by specifying the field name, data type and description.
Make sure you identify the primary key. Print screen the database structure in design
view.

Form

1. Print screen all the form in form view.

Queries
Create queries to generate information below:

1. A list of customers sorted by name and showing just their name, address and
occupation. (Save as Query1).
2. A data of FEMALE customer by showing just name, gender and contact number.
(Save as Query2).
3. A list of types of training, name of customer and instructor – sort in ascending order
based on name of instructor. (Save as Query3).

Reports

Create three reports based on the queries that have been produced and display it properly.
