PARENT FEEDBACK - TEACHER, AIDE, COACH, ADVISOR, ETC. FORM

Dear Parent: As part of our employee evaluation system, we are asking parents to provide us information
about how well our staff works with families. Please take a few minutes to respond to the following
statements by placing an “X” in the column which best describes your experience with the teacher named
below. Completed forms should be delivered to the Superintendent’s office, NOT TO THE EMPLOYEE,
NOR TO THE PRINCIPAL’S OFFICE.

“The Wrangell School Board prohibits all “anonymous comments,” whether favorable or unfavorable.
Hence, if you do not sign this form, it will be immediately destroyed. If signed, your comments and your
identity may be made known to the employee.
