As a young person joining a Magic Bus life skills and employability training programme, I agree to the
following:
· If I change my current number, I will share my new mobile number (working and under my care),
with the Magic Bus team as soon as possible
· If there is any change in my home address, I will share my new residential address with the Magic
Bus team as soon as possible
· I will attend at least the minimum (as explained by the Magic Bus Centre team) i % of the course to
be eligible to get a completion certificate and to get an opportunity to get job.
· If I join a new company, I will share the new employment details (employer name, place of work,
designation, and salary) with the Magic Bus team
· I will support the Magic Bus team and their partner organizations, whenever they call me to
understand the current status of my job
· I will give a copy of my joining letter to the Magic Bus team for their records
· I will share my salary slip and bank statement with the Magic Bus team, as proof of my being on the
job, as and when required.
I understand that these requirements will be needed up to 6 months after I get a job.
I also agree to be a part of any study being conducted by Magic Bus or its partner organizations when
needed (this could be 6 months after my training with Magic Bus) and as per my availability.
I am also hereby giving my consent to Magic Bus to use my name, phone number, email and Aadhaar
number to establish and authenticate my identity in various skill development programmes conducted by
Magic Bus and Govt. of India and for authenticating and uploading in Skill India Portal, and WEP portal of
NITI Aayog, etc. Govt of India, which will help you to get information on various skilling and
entrepreneurship schemes and programmes of the Government and other agencies.
I further hereby permit Magic Bus to use the videos, photographs and other formats of media where I am
individually part of and/or in the group to witness, propagate, disseminate, broadcast, and publicize Magic
Bus Livelihood programme outcomes to various media.
I also understand that my personal details and all the information I share will be treated confidentially and
Magic Bus will take care to keep it safe.
I confirm that:
- I have read all of this consent form, or it has been read (or translated) to me
- I have had the chance to ask questions about it
- I have also been oriented about Magic Bus’s Youth Protection Policy
- I understand it
