4 Mark sheet and Pass Certificate of the Degree(+3) or Equivalent Examination.

5 College Leaving Certificate (CLC) from the institution last attended.

6 Original Conduct Certificate from the institution last attended.

7 Five (5) recent colour passport size photographs.

Original ST / SC / SEBC Certificates from revenue officer not below rank of Tahasildar/Additional Tahasildar. For ST
8 applicant, his/her father’s caste certificate may be taken into consideration. Land passbook authenticated by
Tahasildar/Additional Tahasildar may also be taken as a proof of caste in absence of caste certificate.

9 Original Migration Certificate for the applicant from other Boards except BSE, Orissa.

N.B.:(3). Admitted students must submit their final graduation/equivalent certificate to the allotted institution
within two months from the date of admission. Failing to do so or not fulfilling the eligibility criteria as per the
