Slide 7 (Aster) : Moving on to the Application Features.

Slide 8 (Aster) : There are four main features of our application. The first feature is the
symptoms trackers, specifically we will have a blood pressure tracker, a blood sugar tracker
and a pain tracker. In order to monitor the condition of Metabolic diseases, patients are
advised to measure their blood pressure regularly or have routine blood tests to monitor their
blood sugar, triglyceride and cholesterol level. Hence, these trackers are specially curated to
cater to their needs.

The second feature is the access to virtual and in-person workshops. There are three
categories of workshops available. The first one is Skill-building Workshops, examples are
Blood Sugar monitoring workshop and blood pressure taking workshops. Some of our
users/patients may be new to the symptoms and need help with using the medical
equipment like the blood glucose monitor and blood pressure monitors. The second category
is the Mindfulness and Well-being program, for example hosting guided meditation sessions.
Studies have shown that meditation offers a time-tested, scientifically supported means of
managing chronic pain. Some of the patients diagnosed with metabolic disease may live with
pain and discomfort hence we want to encourage them to seek mindfulness meditation as a
healthier alternative to living with chronic pain. The last one is community/family-focused
workshops. We believe that family and community support is essential for our users who are
living with metabolic diseases, these workshops will encourage bringing family and
community together in support for those living with metabolic diseases.

Our third feature is Journaling. It is a social platform where the users can document their
journey with managing or living with metabolic disease, and at the same time, read
inspiring /success stories from peers on the same journey. There will also be an official
social group for each common metabolic disease. Users may choose to join the ones that
are applicable to themselves or sparks their interest.

And lastly, we have the toolkits which are downloadable educational materials regarding
metabolic diseases. This will enable our users to have easy access to factual informations all
in one app.

Slide 17, 18, 19 (DIYA)

So from the features aster mentioned, we can summarize them under the biopsychosocial
spiritual model. this model is used to define health - where health is a dynamic state of
complete physical, mental, spiritual and social well-being, not just limited to the lack of
disease or infirmity. our app aims offer a healthy lifestyle by attempting to address each
aspect of health.
As you can see in our model features shown here,
the social aspect is more educational and media based. These online materials help the
individual find professional support online, and can be a reliable source to use when sharing
one’s condition with loved ones. success stories of other users in the form of journaling help
one find the intimate support from other companions on this journey, and seek hope that
they can find a balance in their own endeavor.
the psychological aspect we’re focusing relies on the connections forged in face to face
interactions in community gatherings and workshops that provide a accessible community
that one can share their concerns and stresses with.
the spiritual aspect heavily focuses on seeking mindfulness that promotes discipline. to
working towards this self awareness, a series of mindfulness practices can be readily
provided in the app.
lastly, the biological aspect. Sufficient skill and knowledge must be attained on how to record
and track one’s health.
Biological concerns with health must involve regular check ups, for which our app can tie in
with clinics and provide an accessible list of their locations.

Thank you Diya. Moving on to the feasibility of our app.

Slide 13 (Shaun)
First of all, we used Healthy 365 as our reference when we were designing our app. This
app is widely used by Singaporeans as they achieve their health goals. You will notice that
our app features are quite similar such as trackers, programme recommendations,
informative resources and so on.

Slide 14 (Shaun)
But how is our app able to reach out to our target audience? By developing an app, we have
already solved the first part of our problem. People aged between 20 to 25 are more familiar
with technology therefore an app would be convenient for them. Secondly, we are looking to
collaborate with health facilities such as hospitals, clinics, health promotion boards and
wellness centres to support our cause and fund our project as well. Ultimately, we would
want to spread awareness not only to our target age group but to the general public also. We
will be promoting our app through physical ads or digital billboards hopefully inspiring and
influencing people to take good care of their overall well-being or health condition.

LIM JIA YI
Now I will be discussing the effectiveness of our app using the COM-B Model of behavioural
change as a framework to explain how it will be able to induce effective change in our users.

First off we provide users the capability to change by handing them the relevant resources
and knowledge necessary to take the first steps into better managing their specific health
conditions. This is done via the skill building workshops that will teach them to cultivate
healthy habits and how to transition away from bad ones.

This also ties into the opportunity aspect of the COM-B model as armed with this new
information users will have the chance to better themselves. Although we can’t control the
lives of our users we can try to accommodate them as much as possible which is why we
strongly believe in providing a wealth of easily accessible and relevant online information
through our app.
And now, onto how we can achieve that ever elusive motivation to change. We aim to build
communities of users with similar conditions on the app where they can share their efforts,
successes and struggles with each other. And it is through these communities and shared
experiences that we hope users will find genuine and long-lasting motivation to continue on
their health journey

Thus with all 3 aspects of the base COM-B model established, users will be able to
effectively change themselves for the better and improve their health.
