Dear Respondents,

Greetings! My name is Michaela Miel R. Siapno, a grade 12-HUMSS A student at Rizal


Region National High School. I am conducting a study entitled, “Literacy Development: The
Effects of Technology Addiction on Literacy Skills Development of Grade 11-HUMSS” ,
which seeks to understand the possible effects of the excessive time spent on using
technology devices of the Grade 11-HUMSS students that may be a factor of their probable
late literacy development.
In this regard, I am asking for your precious time and effort in answering the attached brief
questionnaire, that are important and helpful for the completion of this study.
