CHECKLIST

It is used to collect data based on the observation of a process's behavior in order to


detect trends through the capture, analysis, and control of information related to the process.
Basically, it is a format that makes it easier for a person to collect data in an organized way and from
according to the required standard in the analysis being conducted. The checklists also
known as check or verification, organize the data in a way that they can be used with
ease later.

Steps for preparing a verification sheet:

1. Clearly determine the process subject to observation. The members must focus their attention on
the analysis of the characteristics of the process.
2. Define the time period during which the data will be collected. This can vary from hours to
weeks.
3. Design a form that is clear and easy to use. Make sure all the columns are clearly labeled.
