INTRODUCTION
Forms are documents that allow users to record, transport
and communicate data and information through paper or
electronically.
DEFINITION
Forms, electronic or paper, can be defined as instruments
intended to codify, transmit, communicate, receive and
preserve any type of data, reports and information selected
and useful in an organization.
FORM DESIGN
The organization and process analyst must consider a wide
variety of aspects for the creation and design of forms. Among
the most important we can mention:
Determining the objectives of the form
The specification of the functions to be fulfilled by the
form.
The determination of its content.
The writing of the text.
The design of the form.
The adaptation to the process or procedure.
The format.
The physical conditions of the paper to be used for
printing, such as:

The type and quality of the paper.
The colors to be used.

Objectives of the forms
Forms can have various objectives for which they are
designed.
Some of the various uses that forms can have:
Communicate: when the purpose of the form is to
transmit information or message regarding something.
Take note: when the purpose of the form is to facilitate
the recording of data or facts.
Authorize: when the purpose of the form is to authorize
or give consent to do something.
Certify: when the purpose of the form is to ensure or
affirm the certainty of something.
Send: when the object of the form facilitates the sending
of something to somewhere.
Special forms
They are those who are prepared for a certain type of work
And they present physical characteristics outside the
traditional.
These forms present fundamental differences in relation to the
common ones. For example: the requirements and design of a
special form are more specific and precise and its printing
cost is much higher than the common form.
The preparation of special forms for specific cases is justified.
For example:
When electronic equipment is used.
When the form is issued in several copies and its
contents a perfect record of all copies.
When there is a need to fill out a large number of the
same form in one place and time.
One of the main types of special forms is the continuous one.
Continuous forms are those made in a band or strip, most
currently used due to the use of computer technology.
